Rectangular patches of missing data

We have been running a particular job for about 3 years with virtually no issues. Every now and then the mesh will have a perfectly rectangular patch of missing data in very specific places. What could cause such a perfect rectangle of missing data. We are confident that we are getting enough shots in that area, the photogrammetry is fine, and have plenty of reference points for good transformation. It only happens in a very rare occurrence..
